Pre-employment Requirements Guide (Diploma, TOR, Clearance, IDs, Medical)
Hello, gladiators!
I am writing this to help those people who are lost LOL (like me). Kung paano magsimula at kung saan magsisimula sa pagkumpleto ng mga requirements mo sa trabaho lalo na kung fresh graduate ka. Request ka na agad sa registrar niyo ng TOR after mo maccomplish ang clearance mo kasi ahit hindi man hingin yun sa mga interview mo, hihingin pa rin yun kapag tinanggap ka nila. For the diploma, may ibang school na hindi agad nag rerelease nito and it is best for you to talk with the HR of the company kung ano gagawin.
I aassume ko na may NSO/PSO generated birth cert na kayo at magpaphotocopy ka na ng birth certificate mo (mga limang copy) at ng mga ilalagay mo as beneficiaries. If wala pa, meron nag aayos sa makababa ng tulay Santa Cruz sa kaliwa (Piety yung name nung travel agency) for 255 PHP. Also, wear jeans and shirt (some offices requires you to wear that).
Magsimula muna tayo sa lahat ng dokumento na governement related dahil alam naman natin na mabagal ang proseso sa gobyerno pero through out this process of completing the requirements na pagtanto ko na malaki ang naitulong ng internet para mapabilis ang proseso. Hindi na ako magpapligoy ligoy, straightforward tayo so here:
CLEARANCE
1.) Barangay Clearance at Sedula- unahin mo na yung malapit sa'yo. Punta ka sa Brangay Hall niyo.
↠Requirements: Birth certificate (Pero hindi naman nahinigi yung sakin coz people in a brangay knows everyone who lives there)
↠Fee: 42 PHP
↠Can be claimed right after payment.
↠Reminders: Ipa-photocopy mo na yung Brgy Clearance and Sedula (3-5 copies)
↠Walang pila
2.) Municipal Trial Court Clearance- Punta ka sa munisipyo then ask kung saan ito makukuha.
↠Requirements: Barangay Clearance at ID (nasa process pa yung valid IDs ko so yung school ID ginamit ko)
↠Fee: 50 PHP
↠Can be claimed right after payment.
↠Reminders: have it photocopied (1 pc)
↠Walang pila
3.) Police Clearance- Punta ka muna sa munisipyo para magbayad at makuha ang official receipt tapos punta ka sa pinakamalapit na police headquarters sa may NAWASA.
↠Requirements: Brgy. Clearance, Sedula, and Official Receipt.
↠Fee: 44 PHP (O.R.) and 180 PHP (Police clearance)
↠Can be claimed right after payment.
↠Reminders: May picture taking dito and have it photocopied (1 pc)
↠Walang pila
4.) NBI Clearance- Meron sa Areza Town Center sa Pagsanjan. Mag register ka muna online and set an appointment, follow mo lang yung procedures. Ako ginawa ko yun kaso hindi ko natapos online kasi tinamad ako, so I decided na magpunta na lang and start from there. May "nag aassist"sa kalapit na computer shop.
↠Requirements: Valid IDs or birth certificate
↠Fee: If you registered online and paid it yourself (115 PHP) if nag pa assist ka (215 PHP)
↠Can be claimed right after payment
↠Reminders: May picture taking dito and have it photocopied (1 pc)
↠May maikling pila
GOVERNMENT IDs
1.) Postal ID-You can apply for this in postal offices. Fill out form to be given.
↠Requirements: Birth certificate, Brgy Clearance, and Sedula
↠Fee: 504 PHP
↠Can be received after 3 weeks
↠Reminders: May picture taking dito Puntahan mo na rin sa office for follow up if hindi mo nakuha after 3 weeks yung ID.
2.) Voter's ID (altho ang alam ko di na sila magbibigay ng ID, certification na lang) Fill out form to be given.
↠Requirements: Birth certificate
↠No fee
3.) SSS ID- Employers only need the E1 form or the static information printout kapag first time mo mag apply. Register online para di ka na mag fill out ng form pag punta mo sa office
↠Requirements: Birth Certificate of yourself and beneficiaries
↠No fee
↠You can request for the ID if may 2 valid IDs ka na or if may hulog na yung SSS mo and if na fill out mo na yung Unified Multipurpose ID (UMID )application form. Two months to receive the ID.
4.) TIN- Print ka na lang ng Form 1902 online tapo fill up mo then bigay mo na sa employer mo, sila na bahala. Nagpunta pa ako sa BIR office bibigyan lang pala ng form 😂
5.) Pag Ibig- Register online, paprint mo back to back yung form. MAy mag tetext din sa'yo tapos sundin mo lang yung instructions na sinabi.
6.) PhilHealth- Register online.

↪Hepa A Screening
1.) Holy Family- may package sila, lahat yan included except for Hepa A screening kasi wala sila nun ang meron lang Hepa B.
↠Fee: 880 PHP/ package
↠Remarks: Feeling ko eto na yung pinaka worth it tapos private hospital pa
2.) Laguna Doctors Hospital- wala silang package so individually mo babayaran and wala silang drug test
↠Fee: ↪Urinalysis (80 PHP)
↪Fecalysis (80 PHP)
↪Complete Blood Count (300 PHP)
↪Blood Type Test (130 PHP)
↪Chest X-Ray (300 PHP)
↪Hepa A Screening (600 PHP)
3.) ADMC Clinic- small place and nacreepyhan ako so yung hepa A screening lang tinanong ko kung meron to find an alternative sa LDH na pricey Hepa A screening.
↠Fee: ↪Hepa A Screening (200 PHP)
4.) Provincial Hospital- I don't suggest na dito kayo mag pa medical kasi sobrang daming patient and too busy yung personnel yung prices naman ay hindi nalalayo sa mga private hospitals and wala silang drug test at Hepa A screening.
